INSURANCE PLANS WE ACCEPT

Below is a list of our current participating insurance plans, as of 8/9/2020.  If your plan is not on the list, we also see many patients with out of network benefits.

New Medicare Patients with PCP referral 
We see new Medicare patients with a referral from the PCP (Primary Care Physician).  Simply have your PCP fax a referral to our scheduling department at 817-410-9963 with the reason for referral.  

Unfortunately, we do not accept any Medicare replacement plans.  We apologize for any inconvenience.

Medicare  New patients need a PCP referral.   Medicare replacement plan - and Baylor Scott & White Medicare.

Multiplan/PHCS PPO

United Healthcare EPO/POS/PPO.  UHC Compass plan requires insurance referral authorization from PCP.
